A "PRP facial" is not one procedure. The term covers at least two different treatments that get marketed under the same name and often the same price, and they do different things to your skin. Before you decide whether it's worth your money, it helps to know which one you're being sold.

PRP — platelet-rich plasma — is made from your own blood. A provider draws roughly 10 to 60 mL, spins it in a centrifuge to separate the platelet-rich fraction from red cells, and uses that concentrate. Platelets carry growth factors, and the theory is that delivering them into injured skin nudges fibroblasts toward making more collagen. That's the theory. What follows is what the published evidence supports and what it doesn't.

Two procedures, one name

Microneedling with topical PRP. A pen or roller with fine needles creates thousands of controlled micro-injuries in the skin. PRP is applied to the surface during or immediately after, on the premise that the channels let it penetrate. This is the treatment most often called a "vampire facial."

Injected PRP. PRP is injected into the dermis with a needle or cannula — sometimes as a series of small deposits across the cheeks, sometimes into specific areas like the tear trough or nasolabial folds. Depth and dose are under the injector's control, which is not true of a topical application.

The distinction matters because almost all of the better aesthetic research uses one or the other, and results don't transfer between them. A trial showing microneedling plus PRP beats microneedling alone for acne scars says nothing about whether injected PRP smooths under-eye hollows.

What the evidence actually shows

The strongest signal is in atrophic acne scarring, and it is a modest one. Multiple small split-face randomized trials — where one side of a patient's face gets microneedling plus PRP and the other gets microneedling alone or with saline — have reported greater scar improvement on the PRP side. A frequently cited 2014 split-face trial in the Journal of Cutaneous and Aesthetic Surgery compared microneedling with PRP against microneedling with vitamin C and found better outcomes with PRP; several later trials, including work published in Dermatologic Surgery and Journal of Cosmetic Dermatology, point the same direction. Systematic reviews of this literature generally conclude that adding PRP produces a small-to-moderate additional benefit over microneedling by itself.

The caveats are substantial. These studies are small — often 20 to 50 patients. Follow-up is typically three to six months. Outcome scales are subjective. And critically, PRP preparation is not standardized: platelet concentration, leukocyte content, activation method, and volume vary between studies and between clinics, so "PRP" in one trial is not the same product as "PRP" in another. Reviewers repeatedly flag high risk of bias.

For general facial rejuvenation — fine lines, texture, "glow," photoaging — the evidence is weaker. Much of it is uncontrolled case series with patient-reported satisfaction as the endpoint, which is exactly the study design most vulnerable to placebo effects and to the natural improvement that follows any procedure involving inflammation and diligent aftercare. A handful of studies have taken skin biopsies and reported increased collagen density after injected PRP, which is biologically interesting but is not the same as a visible, durable cosmetic result. Systematic reviews of PRP for facial rejuvenation consistently describe the literature as low quality and call for larger controlled trials.

For under-eye hollows and dark circles, evidence is thinner still — a few small trials with mixed results.

And there is essentially no good head-to-head data comparing PRP against the treatments it competes with. Nobody has run a large trial of PRP versus prescription tretinoin, versus fractional laser, versus radiofrequency microneedling. If someone tells you PRP outperforms those, they are not citing a study.

The FDA question

No PRP product is FDA-approved for any cosmetic indication. What the FDA has done is clear certain centrifuge systems and kits — under the 510(k) device pathway — for preparing platelet-rich plasma, generally with narrow labeled uses such as mixing with bone graft in orthopedic surgery. Clearance of the equipment is not approval of the facial.

Using PRP on the face is therefore off-label, which is legal: physicians may use cleared devices and autologous blood products outside their labeled indication using clinical judgment. But it means no regulator has reviewed evidence that PRP facials work, and no agency is monitoring outcomes. The FDA has published general guidance for consumers considering regenerative medicine therapies, including the advice to ask whether a treatment is FDA-approved or being offered under an investigational protocol. For a PRP facial, the honest answer is neither.

The infection-control problem, directly

This has to be addressed head-on, because it is the one PRP risk that has produced documented, serious harm.

In 2018, the New Mexico Department of Health investigated an Albuquerque spa offering "vampire facials." The CDC's published investigation identified a cluster of HIV infections among former clients with no other identified risk factors, and concluded transmission was likely linked to unsafe practices at the facility. Investigators found unlabeled tubes of blood stored improperly and equipment handled in ways that could allow one client's blood to reach another. It is described as the first documented instance of HIV transmission associated with cosmetic injection services in the United States. The spa was operating without appropriate licensure; the owner was later criminally prosecuted.

Separately, several state health departments have issued public notices urging clients of specific medical spas to get tested for HIV and hepatitis after inspections found sterilization and injection-safety lapses.

The procedure itself is not inherently dangerous. Blood handled by one licensed person, labeled, kept with a single patient, using single-use disposables, carries very low infection risk. The danger comes from volume-driven settings where multiple clients' blood is being processed simultaneously, needle cartridges or centrifuge components are reused, or unlicensed staff are drawing and handling blood.

Questions worth asking, out loud, before you book:

  • Who draws my blood, and what license do they hold?
  • Is the microneedling cartridge single-use and opened in front of me?
  • Is my blood labeled with my name and kept separate at all times?
  • Is a physician, PA, or NP on site?
  • Is any part of the centrifuge kit reused between patients?

A good clinic answers these without defensiveness.

The procedure and the downtime

Expect 60 to 90 minutes total. Topical numbing cream sits for 20 to 30 minutes. The blood draw takes a few minutes; the spin takes about 10. Microneedling itself is 15 to 30 minutes and feels like sandpaper vibration — tolerable for most people, uncomfortable around the nose and forehead. Injections feel like small stings.

After microneedling: redness resembling a moderate sunburn for one to three days, sometimes pinpoint bleeding on the day, mild swelling, then flaking or roughness around days three to five. Most people are presentable with makeup by day two or three, though makeup is usually discouraged for 24 hours.

After injections: bruising is the main issue and can last five to ten days, particularly under the eyes. Small lumps that resolve over days are possible.

Sun avoidance for a week is standard. If you get cold sores, tell your provider — facial microneedling can reactivate herpes simplex, and antiviral prophylaxis is routine. People with darker skin tones should discuss post-inflammatory hyperpigmentation risk. Active acne infection, bleeding disorders, anticoagulant use, and a history of keloids are all reasons to pause.

Results and cost

Any immediate glow is swelling. Real change, if it comes, appears over four to twelve weeks as collagen remodels, and most protocols use three sessions spaced four to six weeks apart. Studies that follow patients rarely go past six months, so claims about results lasting a year or more are extrapolation, not data. Maintenance every six to twelve months is typical practice, not evidence-based scheduling.

US pricing generally runs a few hundred dollars per session at the low end to well over a thousand at the high end, with three-session packages commonly quoted in the low-to-mid four figures. Insurance does not cover it. Wide variation reflects geography, who performs it, and which PRP kit is used more than it reflects outcome quality.

The bottom line

If you have atrophic acne scars and are already committed to a microneedling series, adding PRP has reasonable — though not strong — evidence for a modest incremental improvement. If you want general facial rejuvenation, the evidence is thin, and cheaper or better-studied options exist. Either way, vet the clinic's blood-handling practices at least as carefully as you vet its before-and-after photos.
